INITIAL VISIT POLICY: All first-time appointments require a valid card on file before booking. This card will be charged if you No-Show or Cancel/Reschedule your appointment less than 24 hours prior to your scheduled time. Please note that we also have a no-refund policy, which applies to all services provided. LATE/CANCELLATION/NO-SHOW POLICY: Please try your best to be on time so that we adhere to the time allotted to you and respect our time as well as the time of all patients. We understand life happens, and will do our best to extend grace to everyone we serve. If you arrive late, your appointment time will be shortened to accommodate every patient. We reserved your appointment time just for you ! Should you need to reschedule or cancel your appointment, we kindly ask for 24-hours’ notice so that we can schedule another patient. If you cancel or re-schedule less than 24 hours before your appointment you will be charged half (50%) the visit fee. If you completely no-show your appointment without notice you will be charged for the visit in FULL.
INSURANCE & THIRD PARTY POLICY: Our office is not in network with any health insurance providers. However, we do accept Health Savings Accounts (HSA) and Flexible Spending Accounts (FSA) as forms of payment in our office. We can provide you with an itemized receipt for you to submit independently to your insurance company. We do not provide any billing or submission of paperwork on your behalf to your insurance company. You are responsible for payment in full at the time of your visit.
